Losing your passport is a nightmare, but you need to act fast. You have to understand this: the government does not issue a “duplicate” passport. They issue a new passport with a different number and fresh validity under the category of “Re-issue” due to loss.

The whole process is now centered on the online portal and the physical appointment at the Passport Seva Kendra (PSK).

Lost Passport: Re-issue Guide (Online and PSK)

Phase 1: The Non-Negotiables (Police and Affidavit)

If you don’t have these documents ready, your PSK appointment is wasted.

1. File a Police Complaint (FIR): This is mandatory, immediately. You must report the lost or stolen passport at the nearest police station. You need a copy of the First Information Report (FIR) for the application.

2. Obtain Annexure ‘L’: This is the Affidavit for Lost/Damaged Passport. You must get this declaration notarized or sworn before a magistrate (check the exact requirements for your state). This document details how and when the passport was lost.

3. Gather ID Proofs: Collect originals and copies of your Identity Proof, Address Proof, and Date of Birth Proof (Aadhaar, Voter ID, Utility Bill, School Certificate, etc.).

Phase 2: The Passport Seva Portal

This is where you register, fill the form, pay the fee, and book the slot.

• Register/Login: Head to the official Passport Seva portal (passportindia.gov.in).

• The Right Category: Log in and select the “Apply for Fresh Passport/Re-issue of Passport” link. This is crucial: You must choose “Re-issue of Passport” and then select “Lost Passport” as the reason.

• Fill and Upload: Fill the entire form accurately. You’ll need to upload the required documents, including the FIR copy and the Annexure ‘L’ affidavit.

• Pay and Book: Pay the fee online. The fee for a lost passport is generally higher than a regular re-issue. Once paid, the system lets you schedule an appointment at your preferred PSK or POPSK (Post Office PSK).

Phase 3: The PSK Appointment and Verification

The system is now digital, but your physical presence is mandatory.

• Visit the PSK: Print your application receipt (though an SMS with appointment details is also usually accepted). Show up on time with ALL your original documents and photocopies.

• Biometrics: The officials will capture your biometrics (photo and fingerprints). They will verify all your documents against the originals.

• Police Verification: A police verification will be initiated after your PSK visit. This typically happens at your current and sometimes previous addresses.

• Dispatch: Once the police report is received and approved, your new passport will be printed and dispatched by post. You can track the status online using the Application Reference Number (ARN).
